MEMBERSHIPS, OFFERS & REWARDS

Please be aware you must fill and sign the contract before joining our memberships or accessing the rewards or discounts via the app, this will ensure you fully understand the terms and conditions and will ensure that your rights are protected.

Memberships can be paused if there are medical reasons or during preganancy however your monthly payments must continue the treatment period of 12 months will be extended up to one year from notification or membership can be cancelled according to the agreed cancellation terms.

Offer, rewards and memberships are accessed via the patient perfect app, you can cancel this membership at any times with the understanding of the following :-

Cancellation of membership any treatments taken will be charged at their full service price, outstanding balances must be paid before the cancellation of the membership.

If cancellation occurs before a treatment is taken any balance will be transferred to your account and can be used as full or part payment of any service or product offered.

Memberships and their associated treatments and discounts can be changed at any time, however if you have joined a reward package this will remain unchanged until the end of the current membership of 12 months.

You may swap items for the same value without penalty within the membership with the express understanding that you will pay any extra fees incurred at the time of treatment and will forfeit any credit for products of a lesser value.

All memberships will commence from the first payment date and all treatments must be completed 12 months after this date, treatments cannot be banked to be used after this time.

You will be given a discount code which must only be used by yourself, you must not give this code to any other person.  Misuse of this code will result in cancellation of orders and cancellation of your membership.

Rewards, membership benefits and discounts acquired by a consumer cannot be transferred or gifted to another person under any circumstances.

Rewards for new patient referrals are only valid for new customers and cannot be registered as a new customer via the app.

Memberships, rewards and packages may be paused due to medical conditions or pregnancy.  You must advise us in writing, any time sensitive benefits, rewards or treatments expiry can be extended for up to 12 months.  Memberships benefits can be paused or cancelled in accordance with the full terms above.
